When you provide us with your email address, we’ll give you online access to select healthcare records, 24 hours per day, seven days a week.

UHS Health Records OnlineHealth Records Online is a secure, online service from Manatee Memorial Hospital that lets you view select medical records online, from the comfort and convenience of your home or office. All you need is a computer and an Internet connection to see your healthcare records, including:

  • Medications
  • Health issues
  • Completed procedures

You’ll be able to view and download discharge instructions and summaries of the care you received at Manatee Memorial Hospital to provide to your personal physician or other healthcare provider. You may even be able to forward the summary of your care directly to your healthcare provider.



It's easy to enroll

Signing up for Health Records Online is easy. Just provide your email address at the hospital and we will email you an invitation to join. Follow the simple steps and you’ll have access to your health records in minutes.

When you receive your email invitation, follow the steps below to complete the enrollment process:

  • Click on the link included in the email.
  • –Click on the button for the type of account you want to create:
    • Personal account: for yourself (adults 18 and over).
    • A minor’s account: For your children age 13 and younger.
  • –Enter the requested information.
  • –Read the hospital’s Terms of Use and Privacy Policy, and click to accept if you agree.
  • –Click on Next, Create Your Account.
  • –Click I Agree at the Terms of Use and Privacy Policy box.
  • –Click the Create Account button.

Your account will be created and you’ll be taken to the Health Records Online home page. You’ll have access to clinical information about your hospital stay.

A: No. Only you may elect to share summaries of your care with other healthcare providers. We do not share your personal information and strictly follow our Privacy Policy. You may read that policy online or when you enroll in Health Records Online.

Q: Is there a fee to use Health Records Online?
A: No, the service is completely free of charge.

Q: Who can I contact for additional questions or for help?
A: If you have questions about your medical record, please contact the Health Information Management Department at the hospital.
If you have questions about getting access to the portal, please contact the Registration Department at the hospital.
If you have technical questions, please contact technical support at 888-252-8146.
